Do you qualify for financial assistance?

Mt Baker Imaging & Northwest Radiologists offer a financial assistance option for those that qualify.

If any of the below applies to you, you are ineligible for assistance under our current policy and need not proceed with your application: 

• Medicare/Medicare Advantage plan member (high contractual discount plans, which make members ineligible for further discount)
•  Active collections balance on file with MBI/NWR (requires resolution with Merchants Credit Association @ 425-643-2613 before financial assistance eligibility can be considered)
• Residency requirement (you must provide proof of established permanent residency in either Whatcom or Skagit County for at least 6 months)

Please return the following information along with the enclosed application, in addition to all applicable documentation identified on page three off the application:

• Last year’s income tax return
• Pay stubs for the last three months or YTD income information from all employers
• If you are unemployed, you must provide all documentation for any awarded benefits (Social Security income, disability benefit income, pension income, etc.)
