Notification of Data Security Incident
10-30-2025 – On or around January 25, 2025, Northwest Radiologists and Mt Baker Imaging (“NWR/MBI”), experienced a network disruption that impacted certain systems. Upon discovery, we took immediate action to address and investigate the event, which included contacting law enforcement and engaging third-party specialists to assist with determining the nature and scope of the event. A thorough investigation determined that certain information stored on our network was subject to unauthorized access from January 20, 2025 to January 25, 2025. We then began a thorough review of the contents of the impacted data in order to determine the type(s) of information contained within the data and to whom that information related. Once completed, we immediately worked to obtain up-to-date address information in order to provide impacted individuals with notification of this event.
The types of information that may have been contained within the affected data may include your first and last name, in combination one or more of the following: address information, telephone number, date of birth, email address, Social Security number, driver’s license or state identification card number, treatment or diagnosis information, provider name, medical record number or patient identification number, health insurance information, and/or treatment cost information.

In response to this event, NWR/MBI has implemented additional security measures within its network and facilities and is reviewing its current policies and procedures related to data security. Although NWR/MBI has no evidence of actual misuse of information as a result of this event, it is always prudent to review health care statements for accuracy and report any services or charges that were not incurred to the provider or insurance carrier. As a best practice, individuals are encouraged to remain vigilant against incidents of identity theft by reviewing account statements for suspicious activity and to detect errors.
